Sheri Rosen

Sheri Rosen, LMSW, is a dedicated social worker with extensive experience in care coordination and management. Since August 2016, Sheri has served as a Social Worker Care Manager at AgeWell New York, providing essential support to clients. Prior to this role, Sheri was a Care Coordinator at Lower Eastside Service Center, focusing on adults receiving opioid and dual recovery services. Experience also includes working as a Care Coordinator at F.E.G.S., assisting clients with psychiatric and medical disabilities, and as a Community Habilitation Specialist at SCO Family of Services, supporting individuals with developmental and physical disabilities. Sheri's early career involved internships at New York City Children’s Center and Developmental Disabilities Institute, where skills in direct client support and facilitation of social groups were developed. Sheri holds a Master of Social Work (MSW) from Hunter College and a Bachelor of Social Work (BSW) from Stony Brook University.
