Rachel Velasco-Acosta

Rachel Velasco-Acosta, DBH, LCSW, CCM, is a passionate healthcare leader with extensive experience in social work, care management, and program development. As the Co-Founder and President of AgileIQ Group since January 2024, Rachel leverages a wealth of knowledge gained from previous roles, including Vice President and Director of Social Determinants of Health at Monogram Health, where comprehensive care management programs were developed and implemented to enhance health outcomes. Prior experience includes significant leadership positions at Blue Shield of California, Wellpoint Anthem, and other organizations, focusing on innovative care models, behavioral health integration, and case management. Rachel holds a Doctorate in Behavioral Health from Arizona State University and multiple degrees related to social work and management, demonstrating a strong commitment to enhancing patient care and addressing social determinants of health.
