Carolina Sokolowski

Project Coordinator For Program Of All-inclusive Care For The Elderly (pace) at AltaMed

Carolina Sokolowski has a diverse range of work experiences. Carolina is currently working as a Project Coordinator for the Program of All-Inclusive Care for the Elderly (PACE) at AltaMed Health Services, where they assist in developing and implementing procedures for data acquisition and management. Carolina also researches and analyzes data for evaluating project performance and presents results to senior leadership.

In 2020, Carolina founded the Kupuna Project, a personal endeavor focused on creating awareness about Aging/Global Health topics and promoting active and healthy aging. Carolina created materials and strategies to improve family and community connections.

From 2021 to 2022, Carolina worked at Watts Healthcare Corporation as a Women's Health Program Coordinator. In this role, they planned, implemented, and evaluated department programs and grants. Carolina also supervised health education, case management, and community outreach components.

Carolina served as a Design Thinking Advisory Board Member at the University of California, Riverside from 2021 to 2022. Carolina provided strategic reviews and input on credit and noncredit offerings, as well as bridging the gap between industry and academia.

In 2020, Carolina served as the Life Enrichment Director at Northstar Senior Living. Carolina coordinated staff schedules, created events, and designed therapeutic and recreational programs for independent living and the Alzheimer's unit.

From 2017 to 2019, Carolina worked as a Grant Proposal and Project Developer at Personal and Family Development Company in Costa Rica. Carolina launched community health programs, managed speaking engagements, and developed team-building and employee engagement programs.

Carolina also worked as a Cognitive Programs Coordinator at Verdeza from 2015 to 2017. Carolina developed workflows and reports for dementia care activities and facilitated multisensory and intergenerational projects.

Before that, in 2015, Carolina was the CEO and Founder of Sweet Memories Cupcakes. Carolina managed highly customized orders, handled financials, invoicing, payments, and inventory.

Carolina gained professional experience in various roles during their time at Universidad de Costa Rica in 2012. Carolina completed professional practices in human resources at Coca Cola Industries, provided psychological interventions at a Psychological Care Center, and conducted psychosocial diagnosis and preventive approaches in the community.

Throughout their career, Carolina has demonstrated a passion for aging and health-related topics, program development, and fostering connections within communities.

Carolina Sokolowski has a diverse education history spanning various fields and institutions. In 2011 to 2017, they attended Universidad de Costa Rica (UCR), where they earned a Bachelor's degree of Science in Psychology. Following this, in 2017, they pursued further studies at Universidad Latina de Costa Rica, focusing on Human Talent Management. Continuing their education journey, they obtained a Mentorship Certification from the Vital Voices Foundation in 2017, specializing in Business and Social Skills. In 2020 to 2021, Carolina Sokolowski enrolled in the Project Management Institute (PMI) and earned the Certified Associate in Project Management designation. In 2021 to 2022, they attended California State University, Northridge, to obtain a Certificate in Executive Management with an emphasis on Design Thinking in Business Administration and Management. Most recently, from 2022 to 2023, Carolina Sokolowski attended the University of California, San Francisco, where they completed the Managing 2 Leading Program, focusing on Organizational Leadership.
