Patient Care Team

The Patient Care Team at American Health Network, Inc., is dedicated to coordinating and delivering comprehensive healthcare services. This multidisciplinary team includes Patient Care Coordinators, Medical Assistants, Certified Medical Assistants, and specialized practitioners such as Asthma/COPD Educators and Health Coaches. They work collaboratively to ensure patients receive timely, compassionate, and individualized care while focusing on chronic disease management, preventive care, and overall health education.
