Jennifer Thomas

Jennifer Thomas, RN, BSN, has extensive experience in home health care, currently serving as a Case Manager at Gentiva Health Services since August 2016. Jennifer is responsible for assessing patient conditions, formulating and evaluating care plans, and maintaining clinical documentation. Additionally, Jennifer has held positions as a Contract Home Health RN with Ardor Health Solutions since January 2016, and previously worked as a Case Manager at Professional HealthCare Network, LLC, and Professional Healthcare Services, where responsibilities included preauthorization, evaluating treatment plans, and providing direct patient care. Jennifer began their nursing career at MGA Home Healthcare as a Registered Nurse. Educational qualifications include a BSN from Arizona State University and a nursing diploma from Butterworth Hospital School of Nursing.
