WHO PAYS FOR SERVICE?
Medicare, Medi Cal and contracted commercial insurances will pay for services for eligible patients. 

WHO IS ELIGIBLE?
Patients under the care of a physician and have been certified homebound may receive home health services. 

WHAT ARE THE BENEFITS?
Our experienced clinicians work closely with your physician to develop an individualized care plan to keep you safe and independent in your home while you recover from injury, illness or surgery. Our care team provides skilled intervention, teaching and tools to help achieve your personal health goals and disease management.
