Beacon of Hope Hospice provides services to patients in their homes, nursing homes, or assisted living facilities. In addition to providing services to our patients, we also provide a period of respite for the family, if needed. All services related to a terminal illness are covered fully by Medicare & Medicaid, and most insurances.

The Case Management Team at Beacon of Hope Hospice coordinates and oversees comprehensive care plans for patients with terminal illnesses. This team ensures seamless communication between healthcare providers, manages patient and family needs, and facilitates access to hospice services, aiming to enhance the quality of life during end-of-life care. The team includes Case Management Nurses and RNs who provide personalized support and guidance to patients and their families throughout the hospice journey.
