Beacon of LIFE (A PACE Program)
PACE is a "Program of All Inclusive Care for the Elderly". It is a Medicare and Medicaid approved program to support the frail elderly that can live at home safely with the support of the PACE services. PACE is a great alternative to nursing home placement and provides all services in the comfort of your own home. Other eligibility criteria include 55 years and older and live in Monmouth County. Services provided include medical, nursing, social service, home health nursing and aides, rehabilitation, recreational day care, nutrition and transportation to and from the PACE center or to medical appointments. Beacon of LIFE services all of Monmouth County located in Oceanport, NJ and Ocean County located in Lakehurst. NJ. The program is based on meeting the individual needs of an enrolled participant. Enrollment is voluntary and one may disenroll at any time.
