Crystal Lysiak

Crystal Lysiak has extensive experience in the healthcare field, currently serving as a Population Health Manager and previously as a Population Health Specialist at Community Care Partners and Nova Health since May 2021. Prior roles include Medical Assistant/Quality Facilitator at Springfield Family Physicians from November 2018 to February 2021, In Home Care at Senior and Disabled Services from January 2017 to June 2019, and 180 hours Externship at PeaceHealth in late 2018. Additional experience includes various roles providing care at Elder Health and Living and Wiley Creek Assisted Living, as well as extensive caregiving for children from 2009 to 2015. Crystal holds a Medical Assisting Diploma from Pioneer Pacific College (2017-2018) and has completed studies at San Jose Christian College and high school diplomas from Los Banos High School and Sweet Home High School.
