Lisa Yordy

Lisa Yordy, LCSW, has a diverse background in social work, specializing in clinical case management and services for older adults. Currently serving as a Clinical Case Manager at Boulder Community Health since October 2014, Lisa previously worked as a medical social worker at Kaiser Hospice / Sutter Hospice from March 2012 to July 2013. Before that, Lisa held the position of Client Services Manager for Older Adults at Jewish Family & Community Services East Bay from September 2005 to July 2011. Lisa earned a Master of Social Work (MSW) from California State University - East Bay, where studies were completed from 2009 to 2011, and also obtained a bachelor's degree from the same institution between 2005 and 2008.
