We provide hospice services to meet the physical, psychosocial, and spiritual needs of our patients and their families/caregivers. An interdisciplinary group of professionals and volunteers develops an individualized plan of care which includes, as appropriate, the following services: nursing, physicians, home health aides, counseling, spiritual support, therapy, dietary, volunteers, durable medical equipment, supplies, and bereavement services.

The Healthcare and Social Services Team at Bristol Hospice is dedicated to addressing the holistic needs of patients and their families by providing comprehensive and compassionate hospice care. This interdisciplinary team, comprising roles such as Associate Medical Director, Medical Social Workers, Spiritual Care Coordinator, Health Specialists, and Hospice Care Volunteers, develops individualized care plans that encompass medical, emotional, and spiritual support. Their collective aim is to ensure the highest quality of life for patients through coordinated services like nursing, counseling, spiritual support, and bereavement services.
