Bronx Community Health Network (BCHN)
Bronx Community Health Network (BCHN) is a non-profit community-based organization and Federally Funded Health Center that is changing lives every day. BCHN assures access to quality, affordable medical, oral and behavioral health care and related diagnostic and social support services at 20 network health centers, for over 96,000 children and adults, regardless of their ability to pay, immigration or other status. BCHN engages and collaborates with community, faith, and multi-sector organizations and agencies to harness local assets and resources for innovative community health promotion, education and outreach programs. This dual approach results in bi-directional, seamless care coordination between health centers and surrounding neighborhoods, connecting patients and Bronx residents with a wide array of services and resources to manage chronic disease, overcome social, economic and education challenges, and embrace healthy living. BCHN also works with community partners to advocate for policies and resources that will achieve health equity for racial and ethnic populations with disparate rates of illness, disability and deaths from preventable and chronic diseases.
