Diane Marrone

Chief of Care Management at CARE DESIGN NY, LLC

Diane was named Chief of Care Management for Care Design NY on January 1, 2022, when Partners Health Plan (PHP) and Care Design NY, a Care Coordination/Health Home (CCO/HH) headquartered in Albany which provides care management for over 28,000 individuals with IDD, aligned to form one organization to better leverage the care management workforce and strengthen the expertise in caring for people with IDD. For seven years at PHP, Diane served as first, Regional Director, then Senior Director and finally, Chief of Care Coordination. Her leadership ensured enrolled members received high quality supports and services to meet their desired outcomes and needs. She continues to provide unwavering support to Care Managers, so they have the resources required to care for individuals and families enrolled in the CCO/HH or FIDA-IDD plan.

Her career in the IDD sector spans 25 years. She has held varied positions within the non-profit sector serving individuals with IDD from birth through adulthood. Experience prior to PHP includes the Assistant Director for Intermediate Care Facilities at AHRC Suffolk where she managed staff and oversaw operations necessary to fully support individuals with both developmental disabilities and complex health needs.

She earned a Master of Social Work degree from Fordham University and is a Licensed Clinical Social Worker and Certified School Social Worker. She also holds an Executive Education Certificate in Non-Profit Leadership from Fordham University.
