The Healthcare Service Management team at CDPHP is dedicated to coordinating and enhancing the quality of care for members across various settings. This multidisciplinary team, comprising case managers, clinical pharmacists, informatics analysts, and community health specialists, focuses on integrating behavioral health, outpatient and inpatient care, and ensuring effective resource utilization. They strive to improve health outcomes through patient engagement, data analytics, and collaboration with healthcare providers, while also addressing health equity and fraud prevention within the healthcare system.
