This team consists of professionals such as Child and Family Team Coaches, Foster Care Discharge Planners, and Care Coordinators who work collaboratively to manage transitions from care facilities and ensure that members receive tailored, effective support. They also handle grievances, liaise with specialty courts and justice systems, and strategize on holistic healthcare solutions to improve outcomes for the families and individuals they serve.
