Christian Community Health Center (CCHC) delivers comprehensive primary medical, dental, behavioral health and supportive services that respond to the needs of the communities we serve. CCHC is a 501(c)3, federally qualified health center (FQHC) established in 1991 to address the health care and social service needs of Chicago. CCHC is a leader in care coordination, with programs that provide services across an integrated spectrum of areas including Health & Prevention; Education & Life Skills; Housing & Shelter, and Intervention & Support. We currently operate seven sites throughout the Chicagoland area, including three community health centers, as well as a mobile health van providing a wide range of primary medical care and social services.
