Community Health Services (CHS) is a network of Federally Qualified Health Centers that provide medical, dental and behavioral health care in six counties in northwest Ohio including: Henry, Hancock, Allen, Huron, Wood and Sandusky. CHS operates nine medical offices in the region and our main campus is based in Fremont, Ohio. CHS providers offer a broad spectrum of care in specialties such as Family Practice, Pediatrics, Internal Medicine, Obstetrics, Gynecology, Behavioral Health and Dentistry. CHS is accredited by the Joint Commission and is certified as a “Primary Care Medical Home” or PCMH. This special accreditation means that CHS is recognized as a model for primary care excellence that is patient-centered, comprehensive, coordinated, accessible, and focused on quality and safety. Patients are treated with respect, dignity and compassion. Over the years, CHS has earned the reputation as a leader for excellence in the region and has a proven track record of meeting or exceeding standards set for the health care industry. Dedicated to meeting the needs of typically rural and under-served communities, income is never a barrier to care at CHS. CHS accepts Medicaid, all Medicaid managed care plans, Medicare, and most insurance companies. Self-pay patients are also accepted and a sliding fee schedule is available for qualifying patients based on income and family size. CHS actively supports and participates in collaborative programs designed to enhance access and quality of health care in the communities that we serve. Partnering with local and regional hospitals, health departments, agencies and other organizations, CHS continues to seek out innovative strategies to maximize the impact local and regional health care has on the health and wellness of the community.
