Community Health Services (CHS) is a network of Federally Qualified Health Centers that provide medical, dental and behavioral health care in six counties in northwest Ohio including: Henry, Hancock, Allen, Huron, Wood and Sandusky. CHS operates nine medical offices in the region and our main campus is based in Fremont, Ohio. Partnering with local and regional hospitals, health departments, agencies and other organizations, CHS continues to seek out innovative strategies to maximize the impact local and regional health care has on the health and wellness of the community.
