C. is a medical practice group consisting of physicians and Advanced Practice Clinicians, that provide hospitalist, Extensivist and primary care services to patients at affiliated acute care community hospitals, long-term acute care hospitals and nursing homes on the northside and near west suburbs of Chicago.
