Jan Campbell, RN, BSN, CDP, CMDCP, is a seasoned healthcare professional currently serving as an RN Chronic Care Management Coach at CircleLink Health since June 2020. Prior experience includes roles as Home Health Director/Director of Nursing at National Nursing & Rehab from March 2019 to March 2020 and Regional Director of Home Health for Texas at THE MEDICAL TEAM from July 2014 to July 2018.
