CCP serves 100,000+ members enrolled in Medicaid, Florida Healthy Kids, sponsored programs, and self-insured employee health plans. CCP is owned by Memorial Healthcare System and Broward Health. Community Care Plan maintains accreditation and has earned Multicultural Health Care Distinction for its Medicaid Health Plan by the National Committee for Quality Assurance (NCQA). CCP is also fully accredited by the Accreditation Association for Ambulatory Health Care (AAAHC) as a Health Plan. MISSION To promote healthier communities.
