Position Title: Community Health Worker (Promotoras)

Department: Community Relations/Outreach

Position Reports to: Director of Public Policy and Community Affairs

Position Summary: Under the supervision of the Community Health Worker Manager and Supervisor, the Community Health Worker/Promotora will be responsible for supporting community members by raising health awareness and well-being through outreach and engagement events. Promotoras will serve as the bridge between its members and healthcare access for all. CHW will build and maintain broad relationships in the community to promote healthy behaviors and provide invaluable resources to improve health outcomes, educate and advocate for better results.

Responsibilities:

  • Engages the community to support with patient enrollment, build and maintain trusting relationships with members and advocate for their healthcare needs.
  • Proper knowledge of community agencies and resources available.
  • Support community initiatives, projects, and activities that seek to better social and health issues in the community.
  • Acts as a liaison between patients/families and health providers by following organization’s values and mission.
  • Provides patient education at external outreach events and assists with the delivery of community resources, including health insurance, housing, food, and quality care information.
  • Provide basic and informal counseling, support, and follow-up with the community.
  • Communicate effectively and professionally when engaging with the public
  • Work collaboratively with internal departments as well as community-based agencies.
  • Ability to organizing resources and establishing priorities.
  • Connect people to health care and social service resources.
  • Assist to build understanding of services available to support healthier decisions and lifestyle choices.
  • Deliver health information using culturally appropriate terms and concepts.
  • Performs all other duties as assigned including participation in special projects and research.
  • Participate in Clinica's activities and events as needed.

Qualifications/Requirements:

  • Demonstrate commitment to serving underserved population

  • Strong verbal, written, and interpersonal skills; ability to display empathy for other.

  • Written and oral fluency in English and Spanish

  • High school graduate or equivalent required

  • Sensibility to work with diverse cultural, social, ethnic and demographic groups

  • Ability to work well in a team setting and independently.

  • Strong follow-through and relationship building.

  • Ability to manage events and projects, coordinating multiple priorities, deliverables, and timelines.

  • Proficient in Microsoft Office and familiarized with the use of electronic gadgets like tablets ands smartphones.

  • Experience working in a community-based setting for at least 1 year preferred.

  • Collaborative and team oriented.
