Catherine Flanery is a dedicated Care Coordinator at Community Connections, Inc., where since February 2015, responsibilities have included providing person-centered care for over 50 adult mental health consumers, developing comprehensive treatment plans, and coordinating with healthcare providers to enhance individual health outcomes.
