The Healthcare Professionals Team at MyCHN is dedicated to delivering comprehensive and high-quality care to patients of all ages. Comprising various roles such as Psychiatric Nurse Practitioner, Behavioral Health Program Manager, Pediatric Dentist, Family Nurse Practitioner, and support staff, the team collaborates to provide a range of services including primary health care, women’s health, behavioral health, and dentistry. They work together to ensure patient-centered care adhering to PCMH principles, while also contributing to the development and improvement of the clinic's open-source electronic health record system.
