We are a non-profit Medi-Cal managed care organization, providing business administrative support for community health centers to focus on what matters most - patients. Our Community: CHCN represents eight community health centers in Alameda and Contra Costa counties in 80+ locations with 450 primary care providers and 1,100 specialists, bringing 141,000+ health plan members the best care at the right time. Our member health centers are Asian Health Services, Axis Community Health, La Clinica de La Raza, LifeLong Medical Care, Native American Health Center, Tiburcio Vasquez Community Health Center, Tri-City Health Center, and West Oakland Health Council. What We Do: Data analytics, case management, specialist contracting, insurance processing, advancement of clinical best practices, and workforce development are what we do to drive better health outcomes for patients with the most complex needs. Our History: CHCN was formed by our member health centers in 1996 to support health centers in a rapidly expanding managed care environment. Its sister organization, Alameda Health Consortium, was founded by community health centers in 1974 to support advocacy, health insurance enrollment, and serve coordination for uninsured and low-income communities.
