- CHDC is a non-profit, community health center created through the generous funding by the Pottstown Area Health & Wellness Foundation along with various other local businesses. - CHDC provides the following healthcare services - Medical, Dental, Vision, Pediatrics, OBGYN, Behavioral Health, Medication Assisted Treatment, Physical Therapy, Podiatry, and Medical Dispensary. - Created to provide accessible, affordable and quality health care to all – which in turn reduces the number of emergency room visits. - Began accepting patients October 2008 with 39 patients during the first three days to a current patient base of over 17,000 (medical & dental patients). - Organized to become the Patient’s Medical Home for preventive care to promote overall well-being. - Free Van/Shuttle service available for our patients with pick-ups scheduled five days a week! - Insurance or no insurance, we accept everyone as patients! Most insurance plans accepted! Sliding fee must be offered to every patient, even if they deny (must sign a form). - Services and sites in scope: Family medical services that include women’s health exams, behavioral health, flu shots, on-site lab services and preventive screenings. - Pediatric care with same day appointments for sick visits, routine physicals and immunizations. - State-of-the-art Dental facility that serves children and adults. - Healthcare discounts for those who qualify. - On-site Case Manager and Referral Assistance available. - Free transportation for CHDC patients (must sign release form). - Expanded evening and weekend hours. - Staff on hand that speak fluent Spanish in order to alleviate any language barriers. - Quality care for the entire family!
