Amanda Westfall-Sell started their work experience in 2020 at St. Peter's Health where they held the role of Manager of Complex Care Management. In this position, they were responsible for managing the transitions of care from inpatient hospitalization to outpatient care. Amanda oversaw nurses and social workers who provided a safe discharge for community members and led a team that aimed to improve the health of patients to avoid admissions and reduce emergency room utilization. Amanda worked closely with hospitalists to establish safe and timely discharges by utilizing community resources to support patients in their homes whenever possible. In 2022, they joined Community Medical Center as the Director of Care Management. No end date was provided for this role.
