Community Hope
Victoria McSweeney currently serves as a Grant & Per Diem Case Manager at Community Hope since March 2024, providing comprehensive case management services, including housing stabilization and advocacy. Prior to this role, Victoria worked as a Mental Health Case Manager at High Point Partial Care LLC from January 2021 to December 2023, facilitating group counseling, developing individualized recovery plans, and coordinating care with various service providers. Victoria also has experience as a DDD Support Coordinator at DAWN Center for Independent Living and as a Community Habilitation Specialist at ACDS, Inc. Victoria holds a Bachelor's degree in Psychology from Montclair State University, obtained between 2018 and 2020, and previously attended County College of Morris from 2015 to 2018.
