Dr. Shari Isenberg-Cohen Dnp

Dr. Shari Isenberg-Cohen DNP, APN serves as the Director of Clinical Care at ConcertoCare since November 2022, following a tenure as Clinic Medical Director at Oak Street Health and Director of Quality and Clinical Operations at Longevity Health Plan. With a wealth of experience in geriatric care, Dr. Isenberg-Cohen previously held roles as a Clinical Services Manager at UnitedHealth Group and as a Geriatric Nurse Practitioner in multiple organizations. Dr. Isenberg-Cohen holds a Doctor of Nursing Practice from Rutgers University–Newark, a Master’s Degree in Nursing from Seton Hall University, and dual Bachelor’s Degrees in Registered Nursing and Economics from Rutgers University.
