Statewide Director Of Acute Care Transitions at Direction Home Akron Canton Area Agency on Aging & Disabilities

Erin Lippe has extensive work experience in the healthcare industry. Erin started their career in 2000 as the Director of Complex Care at Direction Home Akron Canton Area Agency on Aging & Disabilities, where they managed a team of Case Managers responsible for coordinating care for the Ohio Home Care Waiver Program recipients in the Akron/Canton region. Prior to this role, Erin was the Project Manager for the Community-Based Care Transitions Program (CCTP), a nationwide initiative focusing on evidence-based Health Coaching to reduce hospital readmissions for Medicare patients. Direction Home was recognized by the Centers for Medicare and Medicaid Services (CMS) as one of the most successful programs of its kind in the United States. In 2003, Erin joined Summa Health as a Staff RN, where they worked until 2015.

Erin Lippe attended Kent State University from 1994 to 1999, where they obtained a Bachelor of Science in Nursing (B.S.N.) with a focus on Registered Nursing/Registered Nurse. No additional certifications have been mentioned.
