Melinda Moore is an experienced professional in the field of social work, currently serving as an In-Reach Care Manager at Trillium Health Resources since February 2024. Prior to this role, Melinda worked at Eastpointe Human Services from June 2019 to February 2024 as an I/DD Care Coordinator/Assessment Team, and from September 2016 to June 2019 as a Transition Care Coordinator, focusing on assisting members with transitions to independent living.
