Tom Reiter

EVP, PACE Operations at Edenbridge Health

Tom Reiter has worked in healthcare for nearly 30 years in a variety of settings, including mental health, geriatrics, and integrated healthcare models. Tom has extensive experience working with capitation and risk through the PACE model, in addition to participation in developing an Accountable Care Organization (ACO) and medical homes.

During his 18 years leading PACE programs, Tom served as Executive Director of Gary and Mary West PACE of Northern San Diego, where he led the development of West Health’s first PACE program; and as President and CEO of Rocky Mountain Health Care Services (RMHCS), where he led the organization’s development and expansion. While at RMHCS, Tom oversaw growth of the PACE program from 143 to 500 participants, the renovation of a 24,000-square-foot building to become a state-of-the-art PACE facility, and the development of a transportation LLC that served PACE and other programs within RMHCS and the community. Prior to RMHCS, Tom led the PACE program for the Cambridge Health Alliance in Cambridge, Massachusetts.

Tom plays key roles with the National PACE Association (NPA), including speaking at multiple conferences and currently serving on NPA’s Board of Directors. Tom also serves as faculty for the PACE 2.0 Initiative, which is supported by the Gary and Mary West Foundation and the John A. Hartford Foundation, and focuses on exponential growth of the PACE model.

Tom is deeply invested in working with communities to evolve services and systems to support their residents’ healthcare needs. He has also served on multiple boards including Community Health Partnership, Rocky Mountain Options for Long Term Care, Innovations in Aging, and The Foundation for Senior Wellbeing.
