Jake Grundstein

Jake Grundstein currently serves as the Head of Clinical Operations at Eternal since March 2025 and is a Guest Lecturer at New York Medical College, teaching graduate-level courses on value-based care and healthcare operations. From December 2023 to March 2025, Jake held the role of Expert at Primary Venture Partners, providing strategic guidance to investment teams and founders. Prior experience includes serving as Vice President of Clinical Operations and Value-Based Care at Limber Health from July 2021 to March 2025, where responsibilities included negotiating provider and payor contracts and analyzing industry trends. Jake's earlier roles encompass Value Manager, Clinic Manager, and Advanced Clinician at the Hospital for Special Surgery from July 2015 to July 2021, as well as Physical Therapist at Mount Sinai St. Luke's and Mount Sinai Roosevelt from March 2013 to June 2015. Educational credentials include a Master of Science in Health Care Transformation from The University of Texas at Austin, an MBA in Strategic Healthcare Management from Hofstra University, and a Doctor of Physical Therapy from the University at Buffalo.
