Susan Weaver, MD, is a seasoned healthcare executive with a unique blend of clinical and business expertise across both provider and payer sectors. Currently serving as a Senior Advisor at Frazier Healthcare Partners and holds board positions at Acentra Health, AdaptHealth, and Addus HomeCare. As the Founder and President of Transformation Health Partners, LLC, Susan has led initiatives in strategic direction, population health, and business development since 2015. Previous roles include President and CEO of KEPRO, Chief Executive Officer of C3 HealthcareRx, and Chief Medical Officer at Blue Cross Blue Shield of NC, where Susan implemented innovative care models and led healthcare quality improvement initiatives.
