Case Management Team

The Case Management Team at Guild Services provides individualized support and advocacy to clients experiencing mental illness and/or long-term homelessness. They work collaboratively to develop tailored care plans, connect clients with essential resources such as housing and healthcare, and offer ongoing emotional and practical support to help clients achieve stability and improve their quality of life.
