Jose Azar

Jose Azar is an experienced healthcare executive currently serving as the Executive Vice President and Chief Quality Officer at Hackensack Meridian Health since March 2022. In addition, Jose is a Co-Founder and Chief Engagement Officer at both Blue Agilis and the Center for Health Innovation and Implementation Science. Previous roles include Chief Quality Officer, Staff Hematologist/Oncologist, and Medical Director positions at Indiana University Health, where Jose worked from 2009 to April 2022. Jose also served as an Assistant Professor of Clinical Medicine at Indiana University School of Medicine and as Director of Hematology Oncology at Eskenazi Health from 2010 to 2014. Jose holds a Doctor of Medicine degree from the American University of Beirut and another Doctor of Medicine degree from Indiana University School of Medicine.
