Nicole R. Castillo

Nicole R. Castillo, BSW, RSW is a social worker with a diverse background in case management and program facilitation. Currently serving as a Discharge Planner at Halton Healthcare since February 2022, Nicole collaborates with patients and their families to develop and coordinate discharge plans that ensure safe transitions back to the community. Previously, at The Canadian Training Institute, Nicole facilitated group and one-on-one programming for at-risk youth seeking employment. Experience as a Clinical Case Manager at Kinark Child and Family Services involved supporting children, youth, and their families with resource access and safety planning. As a Psychogeriatric Case Manager with LOFT Community Services, Nicole managed a caseload of 52 clients, building strong therapeutic relationships and adapting care plans as needed. Earlier roles at Treetop Trekking included Office Manager, where leadership and employee management were key responsibilities. Nicole holds a Bachelor's degree in Social Work from York University, completed in 2018.
