Hamilton Family Health Team
Jane Ottman currently serves as the Manager of Primary Health Care at Hamilton Family Health Team since December 2023. Previously, Jane held the position of Director of Hospice Care and Quality at Carpenter Hospice from March 2022 to March 2023, and served as Patient Experience Lead at Cambridge Memorial Hospital from August 2019 to March 2022. Jane's extensive background includes over a decade at the Ontario Medical Association as Regional Manager, where responsibilities focused on physician and stakeholder engagement. Additional experience comprises roles at the Ontario Ministry of Health and Long-Term Care, Clinidata, Hamilton Health Sciences, and Sunnybrook Health Sciences Centre. Jane holds a Bachelor of Science in Nursing and a Master's Degree in Health Administration from the University of Toronto.

What is a Family Health Team? In a FHT (Family Health Team) model, family doctors work closely with nurses, mental health counsellors, dietitians, psychiatrists, pharmacists and more to provide team-based care to patients. Who is the Hamilton Family Health Team (HFHT)? HFHT is the largest Family Health Team in Ontario. We have over 165 family doctors and over 250 allied health care providers in our organization.
