Hamilton Family Health Team

What is a Family Health Team? In a FHT (Family Health Team) model, family doctors work closely with nurses, mental health counsellors, dietitians, psychiatrists, pharmacists and more to provide team-based care to patients. Who is the Hamilton Family Health Team (HFHT)? HFHT is the largest Family Health Team in Ontario. We have over 165 family doctors and over 250 allied health care providers in our organization. We serve over half of the population of Hamilton. There are many opportunities for innovation, leadership, and collaboration with other community health agencies at our public-sector organization. Why would you want to be a part of HFHT? • Be surrounded by a diverse group of experts. Here at HFHT, we value collaboration so you will be working with a group of people who can complement and expand your skillset. • Be at the forefront of primary health care in Hamilton. As a leader in Hamilton’s primary care sector, we receive many opportunities and partnerships to better improve patient care. • Never stop learning. In most roles, we provide generous annual professional development packages so that you can continue to grow and refine your professional toolkit. • Care for others’ health without sacrificing your own. At HFHT, we offer competitive benefits packages that could include a Health Care Spending Account. We also offer paid time off (vacation, personal, and sick time) in addition to other benefits. • Enjoy other benefits and work perks: Many roles are eligible for HOOPP, one of Canada’s largest defined benefit pension plans. For most roles, we offer a hybrid working model, meaning that you will be permitted to do some of your work remotely, if you choose. • Expand your role to include organizational and leadership opportunities. Our Green Initiative, Equity, Diversity, and Inclusion Committee, and HFHT Positive Committee are among a few of the groups you can join to work toward the greater good and add some diversity to your day-to-day work.
