What is a Family Health Team?
 
In a FHT (Family Health Team) model, family doctors work closely with nurses, mental health counsellors, dietitians, psychiatrists, pharmacists and more to provide team-based care to patients.

Who is the Hamilton Family Health Team (HFHT)?

HFHT is the largest Family Health Team in Ontario. We have over 165 family doctors and over 250 allied health care providers in our organization. We serve over half of the population of Hamilton. There are many opportunities for innovation, leadership, and collaboration with other community health agencies at our public-sector organization.

The Regional Management Team at Hamilton Family Health Team oversees the coordination and integration of services across various sub-regions. Comprised of multiple Sub-region Managers, the team ensures that healthcare delivery is consistent, innovative, and aligned with organizational goals. They collaborate closely with healthcare providers and community health agencies to enhance patient care and identify opportunities for improvement.
