Harbor Care - formerly known as Harbor Care, Keystone Hall, Southern NH HIV/AIDS Task Force, Healthy at Home, and the Partnership for Successful Living - is an innovative nonprofit community organization helping individuals and families of New Hampshire find solutions to many of life’s most challenging issues. We provide high quality housing and residential services, primary and behavioral health care, substance use disorder treatment, home care, HIV/AIDS care, veteran services, and supports to individuals and families who are experiencing or at risk of homelessness. Our unique model of “housing + integrated care = success” allows us to provide the wide range of collaborative services people need to gain and sustain independence. Each year, Harbor Care provides integrated care and housing to 5,000+ New Hampshire community members statewide, with a focus on the Greater Nashua, Manchester, Salem, Antrim, Plymouth, Claremont, Keene, and southern New Hampshire regions. Our mission is to provide vital services, including housing and healthcare, to all in our communities needing assistance. Our vision is one where everyone gets to live safe, stable, and healthy lives, filled with purpose, respect and dignity. In Greater Nashua over the last 40 years, we have reduced the experience of homelessness in our community substantially. We have dropped rates of unsheltered homelessness and chronic homelessness. We have also achieved an effective end to veteran homelessness and homelessness among people living with HIV/AIDS. These incredible results are only possible because of our incredible network of staff, partners, donors, and volunteers.
