Kelley Frazier

Kelley Frazier currently serves as AVP of Value Based Care for the Mountain Division at HCA Healthcare, a position held since March 2021, and previously held the role of Division Director in the same field. Prior to this, Kelley Frazier worked at HCA Healthcare Physician Services Group from August 2016 to March 2021, where responsibilities included Market Manager of Care Coordination and Value Based Care Coordinator. Earlier experience includes a decade at Holladay Family Practice, serving in multiple roles such as Certified Medical Assistant, Value Based Care Coordinator, and X-ray Technician.
