Dawei Hong, LMSW, possesses extensive experience as a Medical Social Worker, currently serving at Heart to Heart Hospice since August 2023 and previously at VITAS Healthcare from April 2018 to June 2022. Throughout the career, Dawei has conducted psycho-social-spiritual assessments, supported terminally ill patients and their families, and facilitated access to community resources. Additional experience includes working with homeless veterans at the Veterans Health Administration, providing case management and housing support, and serving as a Social Service Coordinator at MidPen Housing Corporation. Dawei also has a background in behavioral therapy, research, and direct support services for individuals with developmental disabilities. Education includes a Master’s degree in Social Work from San Jose State University and a Bachelor’s degree in Community and Regional Development from the University of California, Davis.
