Cherelyn Roberts

Cherelyn Roberts has over two decades of experience in nursing and healthcare management, currently serving as the RN Manager of Community Navigation and Post Discharge Call Center at Holyoke Medical Center since May 1994. In this role, Cherelyn manages a team that includes nurses, community health workers, pharmacists, and a dietitian, focusing on complex care management and addressing social determinants of health. Previously, Cherelyn led initiatives such as the STAAR Initiative, collaborating with the Institute for Healthcare Improvement to enhance care transitions across various healthcare settings. Cherelyn's educational background includes a Master's degree in Healthcare Administration with a focus on Strategic Leadership from Valparaiso University, a Bachelor's degree in Registered Nursing from Elms College, and an Associate degree in Registered Nursing from Holyoke Community College. Additional experience includes the role of ICU/Tele/Medsurg Manager at Wing Memorial Hospital.
