Lisw-Cp Jennifer Consalvo

Jennifer Consalvo, LISW-CP, is an experienced social worker currently employed at Hospice of the Upstate Inc since March 2019. Previous positions include social worker roles at AnMed Health and Tallahassee Memorial HealthCare, where responsibilities included case management and discharge planning for patients. Prior experience as an outreach intern and Go Week leader at Clayton King Ministries involved promoting international outreach trips and serving church communities. Additional experience includes youth ministry internship at City Church Tallahassee and social work internship at Ability 1st. Jennifer holds a Master’s Degree and Bachelor’s Degree in Social Work from Florida State University.
