Monica Fudala

Monica Fudala, MD, FAAFP, currently serves as a Direct Primary Care Physician at ImagineMD since August 2022. Prior to this role, Monica held significant positions including Medical Director at Oak Street Health from October 2019 to July 2022 and served as Chair of the Board of Directors for the Illinois Academy of Family Physicians from October 2019 to October 2021. Additionally, Monica was appointed to the Commission of the Health of the Public and Science at the American Academy of Family Physicians from December 2018 to November 2022. Previous experiences also include leadership roles at NorthShore University HealthSystem and faculty positions at the University of Chicago and Northwestern University. Monica completed a Doctor of Medicine degree at Jagiellonian University and holds a Fellowship in Medical Education Research from the University of Chicago, where Monica also obtained a Bachelor of Arts in Economics and Biological Sciences.
