Diane Jones

Diane Jones has extensive experience in patient care navigation and community health work, currently serving as a Patient Care Navigator/Community Health Worker at Indiana University Health since January 2019. In this role, Diane manages population health case management across various insurance plans, oversees complex care for outpatient management, and assists in transitions of care for patients post-hospital discharge. Prior experience includes roles as a Patient Care Navigator at Evolent Health and Outreach Coordinator at Robert Half - Office Team, where Diane focused on behavioral health case management and transitions of care. Additionally, Diane worked at Brownsburg Health Care Center for 16 years in social services, providing comprehensive support and guidance to residents and families. Diane holds a Bachelor’s Degree in Education from Ball State University.
