Clare Nicholson

Clare Nicholson is an experienced healthcare professional with a focus on palliative and transitional care. Currently serving as the Transition Lead at Keech Hospice Care since October 2022, Clare supports young people with life-limiting conditions as they transition from children's to adult services. Previous roles include Patient Flow Co-Ordinator at East and North Hertfordshire NHS Trust, where support was provided for patient discharge processes, and RPCS Care Co-Ordinator at Peace Hospice Care, coordinating end-of-life care for adults. Clare also has experience as a Paediatric Palliative Care Nurse at Keech Hospice Care and as a Paediatric Nurse at Luton and Dunstable University Hospital. Clare holds a BSc in Nursing from London South Bank University.
