Teresa Hernandez

Teresa Hernandez is an experienced care manager currently with LA Care Provider since May 2020, specializing in managing complex, high-risk members with chronic conditions, mental health issues, and social challenges. Previous roles include medical social work at Satellite Healthcare, where case management for dialysis patients was a focus, and school social work positions at ABC Unified School District and Empire Therapeutic & Creative Services, advocating for student needs and facilitating counseling sessions.
