LIFEPlan is a Care Coordination Organization (CCO), formed in 2018 along with six other CCO’s in New York State to meet the needs of people with intellectual and developmental disabilities by collaborating with service providers through a Health Home model. CCOs are staffed by Care Managers with training and experience in the field of developmental disabilities.

LIFEPlan CCO is a partnership of community-based agencies with a long history of serving people with I/DD, providing advocacy, recreational, educational and vocational services. We share a person-centered focus that improves services and supports through networking with highly trained human service, healthcare and other specialists.

The Leadership Team at LIFEPlan CCO is responsible for providing strategic direction and overseeing the organization's operations to ensure the delivery of high-quality care coordination services for individuals with intellectual and developmental disabilities. This team comprises executives and vice presidents from various departments, including Care Management, Human Resources, Data Engineering, Legal, Finance, Clinical Services, and more. Together, they collaborate to drive organizational goals, enhance service delivery, and maintain strong partnerships with community-based agencies and service providers, all while fostering a person-centered approach.
